Social Prescribing

Social Prescribing connects people to non-medical supports to address issues like loneliness, isolation or stress and  other unmet needs.

Social Prescribing involves understanding the complexities of people’s lives and the inequities they may face. It can help change the circumstances that make people unwell, and empower people to change existing health problems. It can help people to connect and to grow in confidence.

BENEFITS:

  • Reduces loneliness and isolation
  • Improves mental and physical health
  • Decreases the number of GP appointments
  • Enhances community engagement and support

EXAMPLES

  • Joining an exercise group
  • Participating in a community gardening group
  • Attending an art or dance class
  • Volunteering in the community

PROCESS

  • Referral: a healthcare provider identifies a client's needs and refers them to a social prescribing service
  • Assessment: A link worker assesses the client's interests and needs
  • Engagement: The client is connected to appropriate non-clinical services
  • Follow-Up: The link workers monitors the client's progress and provides ongoing support
